Responsibilities include but are not limited to:

Evaluates and treats patients in an Assisted Living Communities predominantly
Evaluates and treats patients in the home setting occasionally
Communicates with ALF, families, physicians, and other health care professions
Reliable source of transportation to commute to different ALF and home locations as needed
Maintains daily, thorough documentation of services in the EMR

Qualifications:

Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy
2+ years Physical Therapy experience
Education and/or Experience: Experienced Therapists
Actively Licensed as a FL Physical Therapist.

Job Type: *Full-time*

40+ hours/ Week

9AM-5PM
